Riprogrammazione metabolica dei Linfociti T regolatori per la terapia cellulare dell’Ipercolesterolemia familiare

L’ipercolesterolemia familiare (FH) è una malattia genetica che si può presentare in forma omozigote o eterozigote in cui difetti genetici a carico dei geni che codificano per enzimi chiave nel metabolismo del colesterolo quali LDL-R, apoB, PCSK9 o LDLRAP1, provocano un drammatico aumento dei livelli di colesterolo LDL nel sangue. I pazienti con Ipercolesterolemia Familiare omozigote sviluppano aterosclerosi coronarica già durante l’infanzia e, se non trattati, muoiono nelle prime decadi di vita per malattia cardiaca, mentre quelli con le forme eterozigote potrebbero andare incontro a eventi cardiovascolari severi come infarto del miocardio già attorno ai 35 anni di età. Mentre le attuali terapie per FH mirano a ridurre gli elevati livelli di LDL colesterolo circolante, il progetto punta a controllare la risposta immunoinfiammatoria nella placca aterosclerotica spesso dovuta a ridotta funzionalità dei linfociti T ad azione regolatoria. Il progetto si propone quindi di riprogrammare il metabolismo di queste cellule per migliorare la loro azione immunosoppressiva e associarla alla possibilità di implementare la capacità di raggiungere la placca aterosclerotica in modo selettivo. Il mix di questi aspetti innovativi rappresenterà un passo fondamentale per lo studio dell’efficacia dell’immunoterapia nei pazienti con ipercolesterolemia familiare da affiancare alle attuali terapie ipocolesterolemizzanti.
